Medical Dermatology

Acne

Acne is an inflammatory condition of the skin that can cause pimples, blackheads, whiteheads, and even cystic lesions. These can occur on the face, neck, chest and back. Contrary to popular belief, people of all ages can suffer from acne. Bacteria, environment, and hormones are involved in the development of acne. There are several treatments (medications and procedures) we can offer to help you manage this common disease.

Eczema

This is chronic skin condition which causes itchy, dry and inflamed skin. The skin may be red, scaly and swollen. Eczema is very common and sometimes runs in families. It can occur in both children and adults. We offer a variety of treatments to help manage this disease, the medications may include creams, ointments and oral or even injections for severe disease.

Psoriasis

People with psoriasis have red or dark, patches on their skin with thick, white scaling. Classically, they are often present on the elbows and the knees but can occur anywhere on the body and commonly on the scalp. Usually these are not itchy but can be for some patients. Some patients with psoriasis also have changes in their nails and problems with their joints. We can help you find the right treatment for psoriasis which can include topical creams or ointments, light therapy treatments or injectable medications.

Fungal Infections (TINEA)

RDA will help clear skin rashes caused by fungus or yeast. These can show up on the skin, the nails, or the scalp. If severe enough on the scalp, the infection may even lead to temporary hair loss, it is important to treat the infection to allow the hair to regrow. Sometimes, we need to do a simple, in-office test to confirm the diagnosis before treating these types of infections called a fungal scraping. Treatments including topical and/or oral medications.

Bacterial infections can also be found on the skin. One example of a bacterial infection is impetigo. Treatment of some bacterial infections require additional testing beyond an examination to select appropriate treatment.

Molluscum

This is caused by a viral infection of the skin. It is characterized by small, skin-colored lesions that may become red and swollen. This condition is most common in kids but can also be seen in adults. We offer several treatments that are effective in treating this common condition, but resolution often requires several treatments.

Warts

Warts are a common condition caused by a virus. Human Papilloma Virus, or HPV, is the culprit and there are multiple strains of HPV causing different types of warts. Here at RDA we use several different treatment methods to treat warts. These interventions almost all require multiple treatments, either here in the office or at home, and patience while your own immune system clears the virus.

SKIN CANCER

1 in 5 Americans will develop skin cancer in their lifetime. The most common type of skin cancer is Basal Cell Carcinoma followed by Squamous Cell Carcinoma and then Melanoma. They may present as a non-healing wound, a scaly patch that never resolves or a changing, bleeding mole. Our physicians are expertly trained to detect and treat all types of skin cancer. We work closely with plastic surgeons, surgical oncologists, medical oncologists, pathologists and radiologists when needed to coordinate and deliver exceptional cancer care for our patients.

Rashes

We are experts in diagnosing all types of skin rashes whether they are from allergens, environmental irritants, reactions to medications, plants or those resulting from a person’s own immune system. Some of these rashes have simple, topical treatment and some require additional testing such as a skin biopsy, to diagnose. Treatments vary widely and expert experience is needed to determine the appropriate treatment for each patient. We work closely with specialized pathologists of the skin (dermatopathologists) to provide the care you will need.

Vitiligo

This is an autoimmune condition of the skin leading to the loss of pigment. This loss of pigment is cosmetically concerning, especially for patients with more natural pigment in their skin. It also increases the risk for skin cancer given the loss of the pigment. This condition can also be associated with other medical conditions, and we will evaluate for those as needed.

Cosmetic Dermatology

These procedures are performed by Dr. Spearman or Physician Assistant Katie.

Neuromodulators

Botox is a popular cosmetic treatment that involves injecting a purified form of botulinum toxin into specific muscles in the face (or other areas). This toxin temporarily blocks the signals from nerves to muscles, causing the muscles to relax. When used for cosmetic purposes, Botox helps smooth out wrinkles and fine lines, especially on the forehead, around the eyes (crow's feet), and between the eyebrows and vertical bands connecting the jaw and neck (platysma bands). The results typically last 3 to 6 months. Over time, the treated muscles gradually regain movement, and the wrinkles may reappear, but regular treatments can help maintain the smooth, youthful look.

Fillers

Facial fillers, also known as dermal fillers, are injectable substances used to restore volume, smooth out wrinkles, and enhance facial features. They are typically used to treat signs of aging, such as sagging skin, fine lines, and hollow areas, or for cosmetic enhancements like plumping lips or contouring the cheeks.

Types of Facial Fillers:

  1. Hyaluronic Acid Fillers: These are the most used type of fillers and include brands like Juvederm and RHA. Hyaluronic acid (HA) is a naturally occurring substance in the body that helps retain moisture and adds volume to the skin. These provide immediate results, is reversible (with an enzyme called hyaluronidase), and is typically well-tolerated by most people.
  2. Calcium Hydroxylapatite Fillers: Radiesse is a well-known example of this type of filler. It contains microscopic calcium particles suspended in a gel.  It provides longer-lasting results (up to 12–18 months), stimulates collagen production, and is ideal for deeper wrinkles and volume loss. It is often used for filling in deeper lines (such as nasolabial folds), restoring volume in the cheeks, and contouringthe jawline.

At RDA, we typically require a consultation prior to treatment with fillers to develop a treatment plan.  The filler is injected with a needle or cannula with minimal discomfort.  Occasionally, a topical anesthetic is used to improve comfort.  Immediate results are often noted but it can take a week or 2 for final results.  Bruising and swelling are common side effects.

Results can last for 6 to 18 months depending on what product is used and where it is placed. Instant, natural looking results can be achieved when this procedure is performed by expert injectors.  Dr. Spearman performs this procedure at RDA.

Kybella

is a prescription injectable medication containing deoxycholic acid, a naturally occurring substance that helps the body break down fat. Kybella (deoxycholic acid injection) is a non-surgical, FDA-approved injectable treatment that reduces submental fat, commonly known as a double chin, by destroying fat cells in the area. For best results and minimal downtime, multiple treatments are often required.

Skin Tags

We safely remove pesky skin tags with minimal pain and downtime. This can be performed with cryosurgery using liquid nitrogen or clipped after the use of local anesthesia.

Seborrheic Keratoses

These benign “wisdom spots” can be bothersome for patients despite their benign nature. We will treat these for you cosmetically if desired. The method depends on the size and/or location of the lesions and can include: cryosurgery, electrocautery and destruction or shave removal.

The Sofwave is an FDA-cleared, non-invasive cosmetic treatment that uses high-frequency ultrasound waves- not traditional lasers- to life, tighten, and smooth the skin.

What It Treats:

Lifts sagging eyebrows, neck, and the area under the chin.

Softens fine lines and wrinkles.

Firms mild to moderate skin laxity.

IPL (Intense Pulsed Light) is a cosmetic treatment that uses broad-spectrum light to target pigment, hair follicles, or blood vessels.  It is commonly used to remove unwanted hair, treat sun damage, minimize redness, and stimulate collagen.

Aesthetic Dermatology

These procedures are typically performed by our highly skilled aestheticians.

DiamondGlow Facial

The DiamondGlow Facial is a non-invasive, multi-step skincare treatment that combines exfoliation, extraction, and infusion of serums to improve skin texture and tone. It’s a popular treatment for those looking to achieve glowing, smooth, and refreshed skin.

  1. Exfoliation: The treatment uses a diamond-tipped handpiece to gently exfoliate the top layer of skin, removing dead skin cells. This process is done with precision, using the diamond tip to effectively slough off the outermost skin layers without causing irritation.
  2. Extraction: The device also has a vacuum-like suction that helps to clear out pores by extracting impurities, dirt, and excess oil from the skin. This is great for those with congested or acne-prone skin.
  3. Infusion: The final step involves infusing the skin with a personalized serum designed to address your specific skin concerns, such as hydration, brightening, or anti-aging. The serum is pushed deeper into the skin using the same device, which helps the active ingredients absorb more effectively for maximum results.

The treatment is customizable, and the serums used are tailored to your skin type and concerns. Some people use it for hydration, others for acne or hyperpigmentation, and some for anti-aging benefits. It provides an instant glow, smooth skin and minimal downtime.  It’s a safe and effective option for most skin types and is often chosen for a “quick fix” before a special event. However, like any facial treatment, results vary depending on individual skin conditions and needs.

Dermaplaning

is a non-invasive skincare treatment that involves using a sterilized surgical scalpel to gently exfoliate the skin's surface. The procedure removes dead skin cells and fine vellus hair (often called "peach fuzz"). During the treatment, our trained aestheticians carefully glide the scalpel over your face in short, precise strokes. This helps to reveal smoother, brighter skin and can improve the texture and appearance of your complexion. Dermaplaning also allows skincare products to penetrate more effectively, and makeup often applies more smoothly afterward. The treatment is typically painless and quick, with no downtime. Some people experience slight redness or irritation immediately after, but this usually subsides within a few hours. Dermaplaning is safe for most skin types, although people with active acne or sensitive skin should ensure this treatment works for them. This treatment is often combined with other treatments to enhance those treatments, particularly prior to Laser hair removal.

Microneedling

-Improving skin texture- addresses wrinkles, fine lines and decreases appearance of pores

-Scar treatment- microneedling can treat superficial acne scars, stretch marks and other scars can be improved.

-Even skin tone- helps diminish sunspots, age spots and melasma

-Boosts skin firmness- by stimulating collagen, it makes the skin appear firmer and more youthful.

In office microneedling is safer than at home microneedling with less complications.  Multiple treatments are typically required for best results.  Downtime is minimal but the skin may be sensitive for a few days and sun protection is very important, particularly after the first week.

Surgical Dermatology

Benign Lesion Removal

benign moles, cysts or superficial fatty tumors can be removed from the skin. Typically this can be done in the office with minimal downtime. Consultation is required to review the treatment plan and coverage for the procdure. The manner in which the lesions are removed is determined based on the size and the location of the lesion and can include: Shave removals, punch excision or elliptical excision.

Skin Cancer Removal

Dr. Spearman has extensive experience in the treatment and prevention of skin cancer including:

  • Electrodessication and Curettage- this is a method used to treat superficial skin cancers where the skin is anesthetized and a special instrument is used to scrape away the cancerous cells. Heat cautery is used to help stop bleeding and as another method of destruction. The area typically heals on its own with specialized wound care.  This method typically has minimal downtime and restrictions.
  • Excision- this is a form of treatment for larger and more invasive skin cancers. This is still performed in the office. The area is marked and anesthetized and the affected area of skin is surgically removed. Usually a deep layer and a top layer of sutures are placed to close the resulting defect after removal.  Some restrictions are involved depending on the location and the size of the lesion.  Specific wound care instructions are given and reviewed with you prior to leaving the office. Pre-operative instructions are given prior to the procedure.

Laser Dermatology

NeoDerm by Aerolase

is a medical-grade laser treatment that uses 650 nm Microsecond Technology to address various skin concerns, including acne, hyperpigmentation, skin laxity, and more, in a safe and effective manner for all skin types. This particular treatment is only available at RDA here in NE Indiana.

Treats:
– Acne and Acne Scars
– Hyperpigmentation and Melasma
– Skin Laxity and Fine Lines
– Rosacea and Redness
– Spider Veins and Broken Capillaries
– Psoriasis and Nail Fungus
– Unwanted Hair

This procedure has minimal downtime and is well tolerated.  Results are best noted with multiple treatments, but the number of treatments depends on the condition being treated.  A consultation prior to treatment is typically required to assess the condition and what is needed.

Intense Pulsed Light (IPL) Therapy

IPL therapy is a non-invasive cosmetic procedure that uses intense pulses of light to treat various skin conditions. The light energy is absorbed by specific targets in the skin, such as melanin (pigment) or blood vessels. This absorption causes heat, which damages or destroys the targeted structures. This can be used for sun spots and red spots on the face, neck, chest and hands. This treatment does carry some risk and careful patient selection is needed.

Laser Hair Reduction/Removal

is a cosmetic procedure that uses laser technology to target and damage hair follicles, inhibiting future hair growth. It's a relatively fast process that can provide long-term hair reduction, especially for individuals with dark hair and light skin. Here at RDA we do have the laser technology to treat those with darker skin types, though more treatments may be needed to ensure safe and effective hair clearance. Maintenance treatments may be needed. This procedure can be performed with minimal downtime with immediate return to activity. Our experienced aestheticians can assess your needs and develop a personalized treatment plan for your needs.
